How to Apply

Your Local Health Integration Network (LHIN), formerly the Community Care Access Centre (CCAC) can help you stay in your own home longer by connecting you with in-home care and support services, as well as services within your community.

If it becomes too difficult for you to live independently at home, they can provide you with long-term care options. To make an application visit your Local Health Integration Network (LHIN) website.
