Diabetes Prevention Programs

We work with community agencies to provide workshops for community members to find out their risk of developing type 2 diabetes and how to prevent it. Healthy eating and physical activity programs are also available, depending on the needs and interests of community members.

Peer Leadership Program for the Prevention of Type 2 Diabetes

The Diabetes Prevention Peer Leadership Program Application for 2017 – 2018 is now available and offered to not-for-profit agencies in Toronto.

We work with agencies to engage and train peer leaders to provide culturally appropriate and accessible health education to adults who are at increased risk of type 2 diabetes. We will provide continued support to peer leaders to deliver diabetes prevention programs within their communities using a community capacity building approach.
