Services for Agencies

Diabetes Prevention Programs

We work with community agencies to provide workshops for community members to find out their risk of developing type 2 diabetes and how to prevent it. Healthy eating and physical activity programs are also available, depending on the needs and interests of community members.

Peer Leadership Program for the Prevention of Type 2 Diabetes

Peer Leadership Program grants are available to community agencies in Toronto. The agencies are responsible for recruiting and supporting their peer leaders. Toronto Public Health works with the agencies to run the program and trains the peer leaders. The peer leaders deliver diabetes prevention education for community groups at high-risk of developing type 2 diabetes. The findings from the Diabetes Prevention Strategy 2009-2012: Evaluation Report (PDF) indicate that peer leadership was effective in reaching community members at high-risk for developing type 2 diabetes.

The application period is now closed for the 2016-2017 Diabetes Prevention Peer Leadership Program. If you have questions, please email preventdiabetes@toronto.ca.
